Referral A request for an evaluation for a student based on educational problems
identified through a screening process. Teachers, counselors, principals, parents
or outside agencies may refer students.
Evaluation
The process of gathering information about the students' abilities, strengths
and needs. This information is used to determine whether special education is
necessary and to help make decisions about the kind of educational programming
that may be needed.
Diagnosis Conference
A meeting to determine the student's educational diagnosis. Information collected
during the evaluation process is reported and discussed. Strengths and needs
are identified to determine whether the student is eligible to receive federally
funded special education services.
Individual Education Program (IEP)
A written educational plan for students eligible to receive special education
services. This plan includes the student's present levels of performance; measurable
educational goals and objectives; the special education and related services
the student is to receive; and dates when these services are to begin and for
what duration and frequency. The IEP is reviewed annually.
Placement
The setting(s) in which a student with disabilities is to be educated/served.
Federal law mandates that the student be placed in the 'least restrictive environment'
(i.e. educated with his non-disabled peer whenever appropriate).
